Summer Slam Foundation Application
The Summer Slam Committee is considering a Foundation to team with for the upcoming Lincolnshire Summer Slam “B” Baseball Tournament.
The baseball tournament takes place at North Park in June of each year.
The Committee shall vote on the charity to raise funds for taking into consideration all ties to District 103 and the Lincolnshire Community.
The decision will be made by a simple majority of those committee members.
Please provide the following information, which will be reviewed by the Committee:
- Name of Foundation
- Whom the funds will be raised in name or remembrance.
- The tie or connection to the Lincolnshire Community.
- Why this foundation would be a good choice or anything you want to emphasize.
